4)  Reunification/Reconciliation therapy (this is a form of family therapy, specifically designed to improve the relationship between a parent and child as a result of separation or divorce) *

*Must be accompanied by a consent order or a court order

Subpoena Information for Attorneys

Submitting a Subpoena
 

●  Our security door prevents in-person deliveries. Therefore, we cannot ensure that we will receive a hand-delivered subpoena.

●  All subpoenas must be submitted via email directly to Dr. Tara Egan at: tara.egan@egancounseling.com

●  For those who would prefer to send a paper copy in addition to an emailed copy, our mailing address is:

Record Requests for Minors
 

If your subpoena pertains to a minor’s records, please note that the one of the following must accompany the request:

●  A signed release from both parents OR

●  A subpoena signed by a judge
